Please do not complete this form until you and I have scheduled an appointment and we have agreed to work together.
After we’ve decided to work together, please complete the form below and click submit at least 24 hours before your first session. All information you share is confidential. If you have any questions, please contact me.

Unless otherwise arranged, sessions are 45-50 minutes in length.
Payment is due at the end of each session and is payable by cash, check or any credit card. If you would like a statement to submit to your insurance carrier, please let me know and I’ll be happy to generate one for you. (It is your responsibility to contact your insurer to determine your coverage and to submit your claim.)
Changing or canceling
There is no charge for canceling or changing your appointment if you let me know 24 hours in advance. This allows me to make the time available to another client. Sessions missed or changed with less than 24 hours’ notice are billed at the hourly fee.
Within reason you are welcome to contact me between sessions and I will get back to you as quickly as possible – usually within 24 hours. If there is an emergency, call your physician or 911.
